I acknowledge that by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge that I have been advised of the facts and matters set forth below and I agree as follows.

I have truthfully presented a valid ID to Queen Street Tattoo NC and have legitimately proved that I am over 18 years of age.

I am free of all communicable diseases.

I acknowledge that it is not reasonable for Queen Street Tattoo NC to determine that I might have an allergic reaction to dyes, pigments or processes used in the making of my tattoo and agree to accept the risk that such a reaction is possible.

I am not pregnant or breastfeeding.

I acknowledge that infection is always possible as a result of obtaining a tattoo, especially in the event that a tattoo is not taken care of properly.

I realize that variation in color and design changes as they relate to my individual body/skin type.

I acknowledge that a tattoo is a permanent change to the appearance of my body.

Obtaining the tattoo is my choice alone and I consent to the application of my tattoo.

I agree to release forever and claims against Queen Street Tattoo NC, for damages or legal actions arising from or connected to the application or healing of my tattoo.

I do not have any abnormal skin conditions such as eczema, scarring, poison ivy, ringworm, etc. that may impede the healing or application of my tattoo.

I acknowledge that I have received aftercare instructions for the care of my healing tattoo.

I understand that certain areas of the body cannot be guaranteed due to the continual regeneration of the skin in those areas, i.e., fingers, toes, feet, hands.

I understand that light pigments do not hold up over time, and that continual exposure to sun affects the average lifespan of the pigments used in my tattoo.

I understand that touch ups are subject to artist’s discretion.

I acknowledge that I am currently sober

I grant Queen Street Tattoo NC the right to take photographs and video of me in connection with getting tattooed at the studio. I authorize Queen Street Tattoo NC to use and publish the photos/videos in print and or electronically. I agree that Queen Street Tattoo NC may use such photos and videos of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising and web content.
